Private/Semi-Private Lesson Cancellation Policy
I understand that there will be a recurring monthly charge to my account and that submission of a 30 DAY NOTICE TERMINATION FORM online is required for cancellation of private/semi-private/Add a parent lessons, and that students/parents must give a 30 DAY NOTICE in order to terminate these classes.

Band & Group Class Commitment & Cancellation Policy
Band & Group programs are group classes that require a COMMITMENT TERM: Fall (September through December), Spring (January through May), and Summer (June through August). To ensure the reliability and commitment level of the student to the group, there will be no withdrawals, cancellations, or refunds for the Commitment Terms of these programs. By registering for these classes, you are agreeing to the Commitment Term of the class. $40 Ticket Charge (5 tickets) pro-rated over term added to monthly tuition fees. Returning Students must register via Customer Portal top of page.

Summer Camp Enrollments
I understand that Full Tuition is required at Registration, unless I have been authorized to receive a 30 Day Installment Option, in which Half Tuition is due now and the other Half in 30 Days. I understand that Payments are non-refundable. Camp Session reservations are required, transferable until May 31st, and subject to availability.

Billing Information
Monthly automatic billing is required. Tuition is due by the first of each month. Monthly flat rates are set whether there are 3, 4, or 5 lesson days in the month. Registration is throughout the year and there is an average of 4 lessons per month. Students that start after the first of the month will have payments pro-rated. Discounts based on full month enrollment. Late fee Policy is $5 per week payments are late. Tuition fees subject to change.

Annual Registration
I understand that registration is through the entire year. An annual Registration fee of $35 (Family Registration $45) will be charged to all students each year (Not including Summer Camp Students).

Release of Liability
Also, I am giving permission for Creative Soul to use photographs of myself (or child if applicable) for promotional purposes.

MakeupPolicy
I understand that canceled lessons on behalf of the student will not be rescheduled or refunded. There will be a makeup class on the last Saturday of each month which will be covering either our Jam Session Class, Music Creation, Songwriting or any other special interest class. I will need to contact the Front Desk to get on this monthly Makeup Class List.
How to schedule a Makeup Class: In order to get on the list for the Make-up class, call or email the Front Desk before your scheduled lesson, and ask to be put on the Makeup Class list. The week before the make-up class you will receive an email with the time of the make-up class as well as the instructor’s name.
In the case that your Instructor misses your lesson: There will usually be a substitute who can continue where you left off from the week before. Sometimes it’s good to have different perspectives from other instructors. In case a substitute cannot be found, you will be notified by the Instructor to RESCHEDULE the class to a time that is convenient for you.

Harvest Christian Academy Students Only (CLASS LOCATION:HCA)
For classes at Harvest Christian Academy only: I understand that registration is through the entire school year (August through May). An annual Registration fee of $45 will be charged to all students each school year. I understand that Private Lessons require a COMMITMENT TERM: Fall (August through December) & Spring (January through May). I understand there will be no withdrawals, cancellations, or refunds for the Commitment Terms of these programs at Harvest Christian Academy. By registering for these classes, you are agreeing to the Commitment Term of the class.
